On June 19, 2023, the Titan submersible, which was heading for a tour to see the Titanic's sinking site, lost contact after its last communication at a depth of about 13,000 feet (about 4,000 m). The Titan's crew consisted of five people: Stockton Rush, CEO of Ocean Gate, the deep-sea exploration tour company that operated the Titan; British businessman Hamish Harding; French explorer Paul-Henri Narjolet; Pakistani businessman Shahzada Dawood; and his son Suleman Dawood.

A large team including the US Coast Guard searched for Titan, thinking that the crew might have survived until the oxygen inside the Titan ran out. However, on June 22nd, a remotely operated unmanned submersible searching the area discovered the remains of Titan on the ocean floor about 400m from the bow of the Titanic. In response to this incident, Ocean Gate suspended all exploration and commercial activities.

Then, on September 16, 2024, the United States Coast Guard's Marine Investigation Board held a public hearing on the Titan accident. During this hearing, the United States Coast Guard released footage taken on June 22, 2023, of the wreckage of Titan that sank to the ocean floor.

As the submarine searches the ocean floor with its light, an object shaped like the tip of a rocket can be seen ahead of it.

This is the remains of Titan's cone-shaped rear section.

The hull of the ship is badly damaged, with the internal framework exposed, and cords and cargo scattered around.

When I turned to the side, I could clearly see the words 'Ocean Gate TITAN' written on the hull.

The hearing is scheduled to run until September 27, and about 20 Ocean Gate engineers and executives are scheduled to testify. The first witness was Tony Nissen, former director of engineering at Ocean Gate. Nissen was hired by Ocean Gate in 2016, but was fired before the 2023 accident because he clashed with CEO Rusch during the development and testing phase. Nissen testified that he had doubts about the safety of Titan.

Tim Catterson, a former Ocean Gate contractor, said there were no red flags in the company's actions on the day of the incident. However, he said he had doubts about the integrity of Titan's carbon fiber and titanium structure and raised the issue with Rush more than 10 times. Rush, however, said he would not listen, saying, 'We have engineers working on Titan, and they all say it's fine.'
