The accident aircraft took off from Portland, Oregon at 17:07 local time on January 6, 2024, but the wall panels were blown off immediately after takeoff, and an emergency landing was made at 17:27. An investigation conducted by the NTSB found that the bolts used to secure the wall panels were not installed correctly.

According to the report released on March 13, 2024, the NTSB requested Boeing for ``video footage from surveillance cameras installed in the workshop'' to clarify the detailed cause. However, the surveillance camera footage had already been overwritten, and no footage remained from the time of the work. As a result, the NTSB has not been able to identify the person who performed the maintenance on the accident aircraft, making the investigation difficult.
